DME STUDENT ATTESTATION FORM 2021
Dear Hybrid Families,
Please be sure to complete a health screening form (Attestation form) everyday that you come in for in-person learning by AM Cohort 8:30am / PM Cohort 1:15pm. If you have any symptoms or a fever over 100 degrees, please stay home. Students will be screened at school if the attestation form is not completed. Students who do not pass the screening will wait in the comfort room until they can be picked up by a family member. Use the QR Code that will be attached to you child’s backpack on the first day. Paper attestation forms will be provided for those with no computer or smart phone.
